It’s called Extra Help, and for many families right here in San Antonio, it is a total game-changer. In 2026, this program is more powerful than ever, potentially saving you over $5,000 a year on your medications.

What Exactly is the "Extra Help" Program?

Technically, it’s known as the Part D Low-Income Subsidy (LIS). But around our office, we just call it "the relief program." It’s a federal program specifically designed to help people with Medicare pay for their prescription drug costs, including monthly premiums, annual deductibles, and those pesky copays.

In 2026, the benefits are truly remarkable. If you qualify for Extra Help, your prescription drug costs are slashed. We’re talking about $0 premiums for basic plans and $0 deductibles. Instead of paying 25% or more of the cost of a drug, you’ll pay very small, fixed amounts: no more than about $5 for generics and $13 for brand-name drugs.

Once your total drug costs reach a certain point: around $2,100 in 2026: you pay absolutely nothing for the rest of the year. For someone managing conditions like diabetes, heart disease, or arthritis, that $5,000+ in annual savings isn't just a number on a page; it’s the difference between stress and peace of mind.

Why 2026 is a Big Year for Savings

You might have heard about changes coming to Medicare thanks to the Inflation Reduction Act. One of the biggest wins is that there is now a $2,000 out-of-pocket cap for everyone on Medicare Part D. That’s great news for everyone!

However, for our neighbors who qualify for Extra Help, the deal is even better. While others are waiting to hit that $2,000 cap, people with Extra Help are saving from the very first day of January. You don't have to wait to spend a couple of thousand dollars before the help kicks in: the help is there every time you swipe your card at the pharmacy.

"Do I Actually Qualify?" (The $24,000 Question)

This is where many people stop. They assume they "make too much" or "have too much in the bank." But the limits for 2026 are more generous than you might think.

For the "mainland" U.S. (including us here in Texas), the 2026 income and resource limits look roughly like this:

  • If you are single: Your annual income should be at or below $23,940, and your total resources should be at or below $18,090.

  • If you are married and living together: Your combined annual income should be at or below $32,460, and your resources should be at or below $36,100.

Wait! Before you close this page because your income is slightly higher: read this: Not everything counts! Your home (if you live in it), one car, your furniture, and even some life insurance policies usually don't count toward your "resources."

Social Security also has certain "disregards." This means they might ignore a portion of your income when they do the math. We’ve helped many San Antonio seniors who thought they were $50 over the limit discover that they actually qualified after all.

The "Double Bonus": Medicare Savings Programs (MSP)

If you qualify for Extra Help, there’s a very good chance you also qualify for a Medicare Savings Program (MSP). If Extra Help is for your meds, the MSP is for your Part B premium.

In 2026, most people pay about $185 or more every month for Medicare Part B. That’s money taken right out of your Social Security check before you even see it. If you qualify for an MSP like the "Qualified Medicare Beneficiary" (QMB) or "Qualifying Individual" (QI) program, the State of Texas actually pays that premium for you.

Imagine getting an extra $185+ back into your monthly budget. In San Antonio, that’s a few extra trips to H-E-B or a nice dinner out with the grandkids. When you combine the savings from Extra Help and an MSP, the total financial relief can be life-changing.
